Practice Hours:
Our hours are Monday - Friday 8:30am through 5:00pm. Laboratory services start at 7:30am.

Now offering appointments every other Saturday from 8:30-11:30am.

Patients are seen Monday though Friday and alternating Saturdays by appointment. We ask you call in advance in order to schedule a convenient time. Every effort will be made to keep appointment commitments. Whenever possible you will be notified if there is a delay. If you cannot keep your appointment, we ask that you notify us as soon as possible at (407)345-0005. In the event that 24 hours notice of cancellation is not provided, there is a $25 fee for general appointments, and $50 for missed physicals.

Emergency Care:
In an extreme emergency dial 911.
If you have an urgent problem, please call our office for instructions. After hours, a physician will be on call for urgent situations. In the event an after hours call must be attended to, there is a $25 after hours service fee.

Prescription Refills:
We ask that all prescription refills be requested at the time of your office visit. Please assess any refill needs prior to coming to the office. We will gladly provide you with refills for up to one year, or your next office visit. Prescription refill requests at any other time will be assessed a $5 service charge. Please allow
48 hours for your request to be processed.

Telephone Calls:
Please feel free to call our office if you have any questions regarding your condition, medication or treatment. Our office staff can also assist you with questions regarding appointments, billing and insurance. We ask that you always spell your name when leaving messages. Please remember our physicians and medical assistants are treating patients during the day - we value your concerns - and will get back to you at our earliest convenience, which may be at the end of the working day.

Please feel free to call our office if you have any questions In some instances a referral is needed for insurance and continuity of care. We ask that you call the office at least 72 hours in advance at (407)345-0005 ext.102

Patient Services:
We participate with many insurance companies. To ensure we are still currently participating in your insurance health plan. We will collect your deductible, co-payment and fees for non-covered services, along with any balances at the time of your visit. We accept cash, checks, Master Card, Visa and Discover. Medical Records :
Your medical records are completely private. To ensure patient confidentiality, and conforming to HIPAA regulations, no information regarding your condition will be released by this office to anyone, including any family member, without your written consent.
